What we could and could not confirm
A delivery note shows what was handed over and who accepted it, with no prices. It settles the argument "I never got the last two". We found no GRA rule that makes a trader write one for goods sold inside Ghana, so this is good practice, not a legal requirement.
In Ghana people also say waybill for the receipt a bus or courier company gives when it carries a parcel. That one comes from the transport company; this delivery note is yours, and the two do not replace each other.
Fill it in two copies. The customer signs yours, and you hand them the other. Photograph yours before the rider leaves.

How to write a delivery note?
Number it, date it, say who sent and who receives, list each item with the quantity ordered and delivered and its condition, then get a name, signature and date from the person who received it. The sample above does exactly that.
What is the difference between a delivery note and a waybill in Ghana?
A waybill is usually the parcel receipt a bus or courier company issues when it carries goods. A delivery note is the sender's own record that the customer received the items. Keep both when you use a carrier.
Does a delivery note have prices on it?
No. Prices go on the invoice. Keeping them off the delivery note means the rider or driver carrying it never handles your prices.
How to get a waybill?
From the transport company that carries the goods; it issues its own. We found no single official waybill form in the sources we read.
Do I need a signature on a delivery note?
It is the point of the note: the signature, name and date of the person who received the goods is your proof of delivery. If they refuse, write it on the note and photograph it.
